Operation Walk 2011 - Day 1, March 24

Day 1 of the Operation Walk 2011 Mission began today with an early breakfast and arrival to the hospital by 6:30 AM. Despite what we thought was an early start, every patient scheduled for surgery the whole week was already there, awaiting our arrival to start the preoperative clinic! Many patients had traveled long distances; one patient told me that she had started her journey yesterday evening and traveled all through the night from her small town in the center of the country to be here on time. This struck me as an indicator of how important the treatment we were about to provide was to all the faces sitting in that waiting room -- many arrived limping in pain or were relegated to a wheelchair because of their symptoms. All of us are eager to see them walk out of the hospital by the end of the week under their own power.

In the preoperative clinic, all patients to be operated on for the week were evaluated by the operating surgeon, anesthesia staff, and physical therapists. Any additional testing or imaging studies were determined, and we finally were able to put a face to the many charts and xrays we have been reviewing over the last many months. The clinic was seamless -- largely due to a veritable army of Dominicam Medical student volunteers. Their generous assistance in doing everything from translating to transport, clinical assisting to organizing and obtaining charts was invaluable. There are upwards of 70 (!) medical students participating in Operation Walk this year, a testament to the enthusiasm that has been generated here in Santo Domingo.
(above, one patient is examined by four of our five attending arthroplasty specialists, Drs. Thornhill, Scott, Burke, and Mattingly.)

Following preoperative clinic, the surgical team met for Grand rounds. All patient charts were presented to the group, and a collaborative review of the xrays by the 5 participating Joint Replacement Specialists was undertaken. Operative plans were made, and the fine points of the proposed treatment were elucidated. This event is an annual highlight, as the educational value of learning from the years of experience of our attending surgeons is tremendous. The event is attended not only by Op Walk team members, but also by Dominican students and staff.
After a quick lunch, the operating began. Proceeding right on schedule, 8 joint replacements in 7 patients (one patient had both knees replaced) were done in one afternoon. Residents from the Harvard Orthopadics program (including myself) had the opportunity to work one-on-one with our attending arthroplasty staff, which was again a great learning experience. We all look forward to the whole week of collaboration, learning, and most importantly, care of our patients.

Operation Walk 2011 - Day 0, March 23

It is the end of arrival day in the Dominican Republic, and despite a long day of travel the team's spirits are high. The majority of us flew down from Boston this afternoon to join Mission Coordinator Roya Ghazinouri, and OR Coordinator Paul Laemmle (who had arrived earlier in the week to make final preparations) at our hotel in Santo Domingo. There were no major travel mishaps, and everyone (and their luggage!) made it safe and sound.
The evening was filled with team meetings over dinner, and was highlighted by a discussion of outcomes data from previous Op Walk missions (2008 - 2010) by Dr. Jeffrey Katz. Data from the initial cohort of patients has shown that they are doing remarkably well -- with drastic improvements in pain and functional measures. Qualitative analyses of patient's perceptions of their illness and the recovery process have also been undertaken, which will assist future providers by creating a cultural context for treating our patients. Further studies are under way, with the goal of ensuring we are providing the highest quality care. (Of note, we were lucky enough to celebrate Dr. Katz' birthday with him tonight -- Happy birthday!) The mission begins in earnest tomorrow, with preoperative clinics and afternoon operative cases scheduled. We are all excited to begin, and feel extremely grateful for the opportunity to be here and working with our Dominican colleagues and patients. Many months of preparation and incredibly hard work by a whole host of people has gone into making this mission a success. Both in Boston and in Santo Domingo team members have spent considerable effort in procuring implants and supplies, organizing cargo, pulling together manpower, identifying and screening patients, and preparing the patient wards, ORs, and cargo rooms for the more than 60 total joint replacement surgeries planned for this week. Photos below give a hint at the sheer magnitude of stuff this number of surgeries requires, and only through the diligence and hard work of our own Roya and Paul is there any semblance of organization...

A special thanks to the many local personnel who have made a incredible effort to prepare rooms, wash mattresses, changing ceiling tiles, and do literally anything and everything to prepare the hospital for Op Walk's arrival and create a space for us to work and care for patients. Below are just a few of these people...

OP Walk Departure: Wednesday, March 23rd

With the start of the 2011 OP Walk surgical mission only four days away, final on-site preparations by Mission Coordinator and Chief Operating Officer, Roya Ghazinouri and OR Coordinator, Paul Laemmle are set to begin in Santo Domingo this afternoon. The pair left from Boston this morning, and will be meeting with our Dominican colleagues to finalize plans and begin the arduous task of unpacking the more than 500 boxes of cargo shipped last month to Hospital Plaza de la Salud.

The rest of the Op Walk team will be arriving on the evening of Wednesday, March 23rd and will hit the ground running with registration, pre-op clinic and the first of the more than 60 total joint replacements to begin early Thursday morning.

We are all very excited to begin our week of collaborative efforts with Dr. Alcantara and the host of Dominican physicians, therapists, nurses, and students who we will be working beside. We are humbled and grateful to the more than 50 patients who will be trusting in us for their care; we are committed to providing that care at the highest level.

There will be much more to come as we begin our mission next week...please return here for daily updates!

Final Preparations Underway

The Operation Walk Boston team is now in the final phase of preparations for their 2011 trip to the Dominican Republic. The last organizational meetings are taking place and 26 pallets of supplies (including 532 boxes and an anesthesia machine) have now been shipped to our host site, the Hospital General Plaza de la Salud in Santo Domingo. There it will await the team's arrival on March 23rd. The Op Walk mission depends on the generous donation of supplies from our many partner organizations, including: the Brigham and Women's Hospital, Cardinal Health, Stryker Artizan Health, AmeriCare, DePuy and others.

Below some photos give an idea of shear quantity of supplies necessary for a mission of this scale:

The mission aims to provide 62 total joint replacements to 47 patients. All patients have been thoroughly screened by a multidisciplinary team made up of surgeons, anesthesiologists, and medical specialists both here and in the Dominican Republic.

Op Walk Fundraiser a Rousing Success!

Operation Walk supporters showed up in droves to rock to the Newgrass stylings of local band, Gone to Seed last Friday night at the Bella Luna Restaurant/Milky Way lounge in Jamaica Plain. The event was a fundraiser for Op Walk's March 23rd, 2011 mission to Santo Domingo, Dominican Republic. More than 150 people made it out to the event, which featured Op Walk physician, Dr. Jeremy Smith as one of the bandmembers. By all accounts, they put on an incredible performance.

Many thanks to Gone to Seed and all of the volunteers who put on a great show Friday, and to the many supporters whose contributions have helped to make the this years trip happen! We couldn't do it without you.

We are now just over a month away from departure, so stay tuned for more pre-trip updates...

Dr. Thornhill Wins Humanitarian Award

On Monday, November 22, 2010, Dr. Thomas S. Thornhill was recognized for his outstanding work with Operation Walk Boston and was awarded the 2010 Humanitarian Award from the Hippocrates Society at Brigham and Women’s/Faulkner Hospitals. As the founder and Chief Executive Officer for Operation Walk Boston, Dr. Thornhill has worked tirelessly to raise funds and lead Operation Walk Boston since 2008.

The purpose of this annual award is to honor the physician whose medical contributions go above and beyond to enrich lives locally or globally, and to improve the patient experience here at home or internationally.

Below is a link sharing some of our memories from our missions to Santo Domingo.

Op Walk Boston 2011 Kickoff

Welcome to Op Walk Boston 2011! I am pleased to report that planning and preparation for the next mission to the Dominican Republic is already well underway. The date of the 2011 mission has been set: March 23rd - March 30th, 2011. With less than five months left to go, much work has already been done to ensure another successful and productive trip.

In late October, members of the Op Walk Leadership team Roya Ghazinouri (Mission Coordinator), Dr. Kanu Okike (Medical Coordinator), and Paul Laemmle (Operating Room Coordinator) went down to the Dominican republic for a preliminary visit. By all accounts, the trip was a resounding success. 75 Potential candidates for joint replacement surgery who had been identified by Dr. Alcantara and the DR team over the past few months were screened for eligibility. 56 patients who met the mission's inclusion criteria were selected to be brought back for preoperative testing. In addition to patient selection, existing cargo supplies were reviewed and cargo needs for March's mission were determined. The visit was greatly aided by Dr. Alcantara and his local team (including many medical students), who were very well prepared and helped to ensure that planning proceeded smoothly.

We are all very excited to be returning (or making our first visit) to the Dominican Republic in March! This blog will be updated regularly with events related to Op Walk as they unfold over the coming months, and we encourage you to visit the blog regularly to learn more about our progress and about how you can help support our efforts.
